Care and Condition Management services are designed to drive patient engagement and coordinate care across the entire continuum. Our care and condition managers received Case Management Accreditation from the National Committee for Quality Assurance (NCQA) and a Certification for Disease Management. Earning NCQA’s case management accreditation is an indication that a Case Management program is dedicated to care coordination, patient-centeredness and improving quality of care: getting the right care to patients.
CARE MANAGEMENT
Care Management is provided to patients who have experienced an event or diagnosis and need help navigating the healthcare system to facilitate appropriate care.
CONDITION MANAGEMENT
Condition Management supports the plan of care for eligible patients identified with diabetes, congestive heart failure (CHF) chronic obstructive pulmonary disease (COPD), asthma, hypertension and hyperlipidemia.
